How to Enroll your Organization

A) Getting Started

1. Read, Print and Sign the Terms and Conditions 
2. Print and Complete the Organization Enrollment Application
3. Include a copy of your Letter of Determination from the IRS
4. Mail: Signed Terms & Conditions, Completed Organization Enrollment Form and Letter of Determination to:

B) How Your Organization Earns

Once your organization is enrolled:

1. Simply encourage your members to register their Smith's Fresh Values Card online at www.smithsfoodanddrug.com, using your organization name and/or Organization number
2. Every time a member shops for groceries and swipes their Fresh Values card, your organization automatically starts earning a donation
3. Members who do not have Internet access can signup by calling               1.800.764.8897       

C) Payment Rules

1. Smith's will pay each school a percentage of the fund equal to the percentage of total earned contributions attributable to that school.
2. Your annual check will be issued to your school in the fall of each school year.

Q.  What is a Letter of Determination? 
A.  A document from the Department of Treasury which verifies that your organization is a not for profit  organization and has exempt status under section 501(c) (3). To request a letter of determination please contact the Department of Treasury at               1-877-829-5500        between 8:30 am – 5:30 pm Eastern Time.
